  1. There are problems with your optimism about physician assistants and variations. One is that it is helping to convert medical doctors into mommys. In rural area esp. the population is aging and therefore finds it difficult to take the measures of diet and exercise. They want pills and assurances, and female docs seem eager to supply them in place of explanation and bigger pictures.

    The second factor I find problematic is “Practice Managers” who are supposed to take the burden off docs but tend to become little tyrants of time limits, interior decorations, and promoters of constant return visits for trivial matters as long as they are billable. They tend to be male and bully the female former nurses.
